If you have an insurance that is not contacted with FootWorks Physical Therapy, you may be eligible for out-of-network benefits. FootWorks Physical Therapy understands the impact of healthcare costs and will work with you to provide affordable care. Claim forms and codes are provided to make submitting your claim as simple as possible.
FootWorks does not participate with Medicare and is unfortunately not able to treat those covered with a Medicare policy.
